For the convenience of the public, the Department strives to provide as many as possible downloadable application forms and other information. These can be downloaded for reference or printed out and used for submission. However, to obtain those forms where payment is required, the applicant will have to visit the nearest Land and Survey Division Office.
The form following are available:
(1) Quotation for Sale Format (in .pdf)
Format for making recommendation for offer in a "Quotation for Sale".
(2) Stores for Write-off Form (in .pdf)
Form for use in requesting for approval in writing-off for used items.
(3) Disposal Certificate (in .pdf)
Form for use in reporting on disposal of used items.
An application form for use by Government Departments or Agencies when applying for a site to erect buildings or to
implement other development projects.
An acceptance form for client to sign, indicating that the site being offered is accepted or other wise. This form is to be signed by the client during the joint site inspection.
